I'm slowly fitting in at my new clinic assignment but it is discouragingly slow with new everything - car, phone, EHR, referral systems, colleagues... MY coworkers say I am doing great and the patients are very appreciative and frequently say that they hope I will stay there. I think the balance is still positive but it is a sad commentary. There are challenges for sure with a new work location.. the core of what you do.. practice medicine.. I suspect you don't have any issues with. The other stuff is a bother I bet. Exactly. Doctoring is fairly consistent wherever one practices medicine but different EHR, staff policies and procedures, insurance rules, and a zillion other "little things" that have nothing to do with medicine are a challenge. There are local and regional variations in some aspects of medicine, but they make sense and a question or two will relieve any confusion: eg, are the colonoscopies done by the GI guys or the general surgeon? I call those "How do we do it here?" questions. I know several correct answers but local amenities may favor one correct answer over another.

TNTC - too numerous to count! There are at least two components - those of the location and those of changes in science.
When I moved away from Denver in 1990 I remember calculating how many people I had met in my life. I think it was more than 100,000 but I don't remember how I calculated it, much less what it might be now.
Yesterday I saw a patient who had also grown up in a military family. Comparing notes, we had lived in many of the same places, mostly not at the same time, though we were both living in Hawaii in 1964 when an earthquake in Alaska necessitated an evacuation to avoid tsunami. It's the little coincidences and connections that occur that count, not the number.

This is one reason I always enjoy going up to LaGrange this time of year. I enjoy the daffodils and tulips popping up and the redbud and dogwood are blooming.

So I am on-call this weekend and just took an "emergency call" from a Home Health speech therapist to inform me that she recommends a swallowing test and a change of all this patient's meds from pills to liquid form. Now patients don't always understand "emergency" calls, but another health professional might be expected to know that Imaging isn't going to do a swallow test on the weekend, and I am disinclined to rearrange all the meds for another docs patients when he's been on them for months and IMHO this could wait until Monday.
I was not told in the interview or the contract terms that this job includes "on call" responsibility. That is unusual because docs do temp assignments to avoid call on nights, weekends and holidays. There are also many good medico-legal reasons to have real employees rather than temps take on call responsibilities.
That has really pissed me off that this was snuck in like that but since I like other aspects of the job, I'll stick with it for a while.

It could be worse. At least this was daylight hours, not between midnight and 0600, and it's a reasonable medical suggestion, although not on a Saturday. However, the Home Health employee should be sending a written report and request to the physician in charge of the patient's care rather than paging the on-call doc for an "emergency.".
